The Care Coordination team at INTEGRIS Health understands how critical it is to treat the whole person. It’s about treating people like human beings, not just patients. By addressing the underlying causes of chronic pain and illness, they aim to break the cycles of suffering and restore hope to those in need of quality health care.
Patient Assistance
Through the generosity of our donors, the INTEGRIS Health Foundation has established funds to support care coordination at INTEGRIS Health Southwest Medical Center. Many patients at this facility represent some of our state’s most vulnerable populations. This program gets to the root cause of patients’ health issues, so their well-being is improved long-term. To date, Care Coordination has used more than $111,000 from these funds to help patients who need it most.
