Golf Classic Pledge Form

Thank you for choosing to partner with us for the benefit of patients, initiatives and programs at INTEGRIS Health.

If you are ready to make your pledge for our 2026 INTEGRIS Health Golf Classic, simply complete and submit the form. A member of our events team will contact you as soon as possible to confirm your pledge.

Please Note: Sponsorship pledges must be fulfilled prior to the event in order to participate. If you are supporting our events as an underwriter, we kindly request that you fulfill your pledge prior to your logo being used in any event promotion. All gifts are non-refundable.

By submitting this pledge form, I acknowledge that this constitutes a commitment to the INTEGRIS Health Foundation. Sponsors are expected to fulfill their pledges prior to the Golf Classic event date of June 15, 2026. Underwriters are expected to fulfill their pledges prior to their logo or name being printed on any event materials, promotional items, or signage. Golf Classic Sponsorship Opportunities

All sponsorships include participation in the tournament.

Tournament Sponsor | $25,000
Includes three four‑player teams, plus one mulligan per player. Your logo will be integrated into the event branding and appear prominently on event signage and all promotional materials, including social media, the website, participant emails (sent to 250+ business leaders), and the Foundation newsletter. You will also receive six additional reservations for the 19th Hole reception and be recognized from the podium. Pending availability, you may golf with the INTEGRIS Health executive of your choice. Tournament sponsors may host a table and share materials with players. 

Eagle Sponsor | $12,000
ncludes two teams of four players plus one mulligan per player. Your logo is prominently displayed on promotional materials and event signage. You also receive four additional reservations for the 19th Hole reception and verbal recognition from the podium. Pending availability, you may play with the INTEGRIS Health executive of your choice.

Birdie Sponsor | $6,000
Includes two teams of four and two additional reservations for the 19th Hole reception. Your name will appear on promotional materials and signage.

Par Sponsor | $3,000
Includes one team of four players. Your name will appear on promotional materials.

Golf Classic Underwriting Opportunities

Your generous support of the INTEGRIS Health Golf Classic as an exclusive underwriter does not include green fees to play in the tournament. If you would like to participate in the tournament with your team, please ensure that you include a sponsorship when submitting your pledge.
